The Sentinel Communities Surveillance project began in 2016 and has been monitoring activities related to how a Culture of Health has been developing in each of 29 diverse communities around the country. The purpose of the project is to understand how a community views and prioritizes health, well-being, and health equity influences the strategy, policy, and investments the community makes to improve health outcomes. This report charts the evolution of those beliefs and practices over the course of the last five years (2016 – 2021).
